The International Neuroblastoma Pathology Committee classifies tumors into 7 types categorized as either Schwannian stroma (SS)-poor, -rich, or-dominant. Clinical risk group designations are based on age, MKI, MYCN amplification, ploidy, chromosomal aberrations and stage. To investigate the relation between tumor behavior, histopathology and angiogenesis we evaluated vascular morphology on H&E, CD31, vWF and alpha- SMA stained sections from over 200 cases. We found that Vascular Endothelial Proliferation (VEP) was present only in SS-poor tumors and regions, and was associated with shorter survival and other high risk factors. We evaluated neuroblastoma xenografts following in vivo treatment with Thrombospondin-1 peptides and/or Valproic acid. Anti-angiogenic treatment abrogated VEP. Our observations describe the histologic counterpart for ‘normalization’ of vessels seen in dynamic leakage studies. Our results suggest that histopathologic evaluation of Neuroblastoma for VEP could refine risk group designation, modify eligibility criteria for clinical trials, and provide insight on tumor response to anti-angiogenic therapy.
